About the Role

JOB TITLE: Direct Support Professional Team Member

SUMMARY:
Full range of personal care to individuals with disabilities.
Accurate mileage of any trips taken.
Homemaker personal care (HPC) activities completed during the shift.
Document, Service Notes, Behavior documentation, Incident reports, Communication logs. Various logs that may be required by medical professionals, behavior specialists, etc.
- Personal Hygiene
- Cleaning on Daily Basis.
- Lifting and Providing Personal Hygiene Care
- Cooking Daily
- You must know how to follow directions
Communicate with co-workers and home managers about concerns or activities completed/need completed throughout the shift.
Maintain required certifications and training (recertification classes offered).
Monitor medical needs of the individual(s).
Provide emergency care, as needed.
Provide behavioral intervention, as needed.
Serve as a role model of appropriate behavior and social skills in the home and the community.
Conduct additional services requested by our consumer or their service team that may improve their quality of life, health and safety.

Requirements

  • High school or equivalent education.

  • Minimum of 1 year of continuous, verifiable work experience in a care or customer service role.

  • Experience working in patient care or with individuals with disabilities preferred.

  • Must have current & valid OH driver’s license as well as personal car insurance with vehicle.

  • Must possess First Aid, CPR, AED & Medication Administration Certifications (if certifications are needed, trainings can be provided).
